  • Clinical and evaluation data: Test results, behavioral observations, progress notes, treatment plans, and session summaries.

  • Website or online form submissions: Information voluntarily submitted through forms, scheduling requests, or inquiries.
     

We only collect information necessary to provide high-quality psychological and educational services.

  • Providing therapy, counseling, or consultation services

  • For minors, records are retained for at least 7 years after the client turns 18.
     
  • For adults, records are retained for at least 7 years after the last date of service.
     

After this period, records are permanently and securely destroyed.

You have important rights regarding your personal and health information, including the right to:

  • Request corrections to inaccurate information
     
  • Request limits on how your information is used or shared
